Leo J. Urbinelli, MD, Board Certified Plastic Surgeon *Top Plastic Surgeon in CNY* Bringing his expertise from NYC and Los Angeles back home to Upstate NY
Your ears might grab attention in a way that feels bothersome. They may stand out too much, appear too large, appear pulled, or even be missing natural anatomy due to prior trauma or injury.
At Premier Aesthetics, Dr. Leo J. Urbinelli does Otoplasty or Ear Surgery in Syracuse, NY, for children, young adults, and adults who want their ears to look more natural and proportionate with their face.
This kind of surgery is not about making you a different person. It is about making a feature less obvious or improving an area that has been bothersome, uneasy, or upsetting for too long.
Dr. Urbinelli looks at cartilage shape, ear position, symmetry, skin quality, and the patient’s age before building a plan.
A careful ear reshaping plan starts with the anatomy. Dr. Urbinelli does not treat every ear concern the same way because the problem may be cartilage, skin, missing folds, stretched lobes, old trauma, or a mix of issues.
At Premier Aesthetics, Dr. Urbinelli uses his plastic and reconstructive surgery and craniofacial surgery background to improve the form of the ear in a way that looks natural from the front, side, and back.
Some patients have been bothered by their ears since childhood. Others come in later after an injury or trauma. Dr. Urbinelli listens first to your concerns, then offers options (if safe, possible, and realistic) and then helps create a plan.
The consultation with Dr. Leo J. Urbinelli is where the real answers start. You explain what bothers you or what worries you about your (or your child’s) ear shape. Dr. Urbinelli examines the ear structure, takes the face into account, obtains a thorough history, and explains what can be done without overcorrecting.
In children, timing matters for appropriateness for surgery, psychosocial well-being, and suitability. In adults, recovery is relatively rapid with inconspicuous scar placement and a natural shape achieved. Dr. Urbinelli walks through the plan in plain language, so families and patients understand the why behind each step.
Depending on the concern, treatment by Dr. Urbinelli may focus on:
Healing is typically rapid, but you must still take adequate time to care for the repair. Dr. Urbinelli gives detailed guides about post-op bandages, post-op limitations, how to sleep, and your follow-up appointments. Swelling and minor pain are common but limited shortly after surgery.
Children may need help avoiding rough play for a bit. Patients need to avoid pressure on the ears while healing. The result starts to show as swelling subsides over days to weeks.
Cosmetic Ear Reshaping Surgery in Syracuse, NY can assist people who feel their ears draw notice away from the rest of the face. This might be due to ear prominence, lack of balance, big lobes, or a form that just feels out of place.
Dr. Urbinelli approaches cosmetic ear surgery with respect for natural anatomy and an artistic eye. He knows ears should not look pinched or overly corrected. They should sit naturally. They should still look like ears, just less distracting.
You may be a good candidate if:
Some concerns are small; others may require full reconstruction of missing pieces of the ear (cartilage, skin); however, each case is approached with compassionate, precise care to achieve a beautiful result.
Otoplasty ear surgery in Syracuse, New York, is often discussed for children, yet adults seek it too. Dr. Urbinelli treats both groups with a different kind of planning. A child’s tissue, growth, comfort, and school setting all matter. With adults, the concern may be decades old, or it may be a newer issue from piercings, aging, or trauma.
At Premier Aesthetics and Plastic Surgery of NY, Dr. Urbinelli gives each patient a plan based on their medical background, age, the anatomy of the ear itself, and the person attached to it.
Patients or parents often ask Dr. Urbinelli about:
The emotional side can be heavy, especially for kids and even adults with ear cosmetic issues. Dr. Urbinelli keeps the visit calm, supportive, and an open dialogue to cover options, expectations, and alternatives (if any).
Premier Aesthetics gives patients space to ask questions. Dr. Urbinelli explains what is possible, what is realistic, what is not needed or safe or will not produce a good result, and what kind of result would look natural. His experience and transparency matter with otoplasty because overcorrection can be just as noticeable as the original concern.
Dr. Urbinelli’s reconstructive background helps with more complex ear concerns, too. Large ear defects, severely stretched earlobes, and congenital missing ears (anotia or microtia). They need judgment and expertise.
Patients usually want their ears to stop being the first thing others notice. Dr. Urbinelli works toward a goal where the ears are just part of the facial/head overall picture, not taking “center-stage”.
The ears sit at the edge of the face, but they still affect the whole look. If they protrude, the face can feel less balanced from the front. If the lobes are torn or stretched, jewelry may not sit right.
Dr. Urbinelli treats ear surgery as part of facial balance. The ears should frame the face quietly. They should not compete with it.
This is why planning matters. The angle, fold, lobe, cartilage, and scar location all affect the final result. Dr. Urbinelli studies and plans these details long before surgery.

Otoplasty or ear surgery reshapes or repositions the ears. Dr. Urbinelli may use it to correct prominent ears, uneven ear shape, or certain cartilage concerns.
Children may be evaluated once the ears are developed enough (7-10 years of age typically) but before the concern diminishes the child’s comfort or confidence. Dr. Urbinelli also performs otoplasty for adults.
No. Otoplasty may also help with asymmetry, weak ear folds, stretched earlobes, torn lobes, or shape concerns after injury or missing anatomy from birth.
Recovery varies, but swelling and tenderness are common early on. Typically, days to weeks later, patients look and feel great. Dr. Urbinelli gives detailed aftercare instructions and will check on your healing during repeat follow-up visits.
Incisions are often placed where they can’t be seen (behind the fold or in a shadow). Dr. Urbinelli plans incision placement carefully, though scar healing may vary by person.

After doing a thorough research for a plastic surgeon, I had a consultation with Dr Leo Urbanelli I immediately felt at ease talking to him about what I was hoping to accomplish. His depth of his knowledge and skill as a surgeon became obvious as he discussed the variations of my options. Dr. Leo would look at a particular area of my face and tell me what he could do and how it would look. He spent a great deal of time with me and answered all my questions and concerns. We both decided what would be the best procedure for me. I am just 8 weeks post surgery and my face, neck and brow lifts came out so much better than I could have possibly imagined. The stages of my recovery were exactly what he said they would be. I knew what to expect and how I would feel as my recovery progressed. He truly takes great pride in his work and cares deeply for his patients. He has always been available day, night and even weekends. He is an incredibly talented surgeon and the most thoughtful person you will ever meet. He is a true artist.![]()

I can’t say enough wonderful things about my experience. I was extremely nervous going into this, and from the very first appointment, he made me feel completely at ease. He took the time to listen, explain everything clearly, and truly understand my concerns. He treated a cyst-like spot for me and repaired an area that kept reopening, something that had been both frustrating and worrisome. The results have been amazing, and I couldn’t be happier. Not only did he fix the issue, but he did so with such care and precision. What really stood out to me was his kindness and attention to detail. At one point, he even noticed and studied some of my old scars, which honestly made me giggle—it’s clear he sees things through a very skilled and thoughtful lens. His passion for what he does really shows. I’m so grateful I trusted him with something I was so scared to do. I would highly recommend him to anyone looking for a talented, compassionate surgeon.![]()

I met with Dr. Leo in 2025 with anxiety and hesitation, I left with hope. During my visit he was patient, empathetic, and kind. He brought to light an issue of Lipedema that has plagued me for decades. After two procedures, I can say his professional skills and bedside manner are unmatched. I’m looking forward to a life of no limitations and more confidence, while looking better than I have in years. Thank you!![]()

Dr. Urbinelli is the best! He performed a breast reduction on me in December 2024 and choosing him as my surgeon was the best decision I have ever made. My results are exactly what I asked for and my self-confidence has increased tremendously. I am now able to live an active lifestyle without being uncomfortable or in pain. I consulted with other surgeons and none of them made me feel as confident and comfortable as Dr. Urbinelli did. He treats his patients like family and it is so evident that he loves what he does. Anytime I had a question or concern, Dr. Urbinelli got me in the office within one or two days to address my concern (and often offered to do virtual appointments so I didn’t have to take too much time out of my day to travel to the office). Upstate NY is so lucky to have a plastic surgeon as knowledgeable, professional, and talented as Dr. Urbinelli. I would recommend him to anyone!![]()

I cannot praise Dr. Urbinelli and the staff of Syracuse Plastic Surgery enough for the care which was given to me during my recent surgery and recovery. Dr. Urbinelli is caring person and an artist at what he does. He was responsive to all my needs especially due to an unforeseen issue during my recovery. Dr. Urbinelli even made a house call to teach my sister how to dress my wound. I traveled far from Las Vegas to Syracuse, NY because he is a reconstructive facial specialist. I am so pleased with my results. I highly recommend Dr. Urbinelli for any reconstructive or plastic surgery you may need.![]()

I waited as long as possible to write this review because I was holding out hope that I could leave a rave, 5-star review, but I can't. I had a revision surgery in August of 2023, in an attempt to correct my breathing and fix countless issues the first surgeon left behind. Knowing this surgery wouldn't be a small task, I tried to remain patient and hopeful with Dr. Urbinelli and team that all would go great. It didn't. My breathing is the same, if not worse, than before (which is terrible). It feels like I am suffocating all day, every day. The aesthetic of my nose has gotten worse (fat, asymmetrical, twisted, etc.). The columella scar is still visible. Swelling refuses to go down. I still have a septal perforation, collapsed nasal valves, deviated septum, and enlarged turbinates. He used a cadaver rib, which seems to be curved and feels rock hard, which is extremely annoying. I have a foul odor that has been persistent since surgery. And when I can't smell that, my sense of smell is practically non-existent. The entire process has been miserable and extremely frustrating. I did everything (and more) that I was told. Taping, Vaseline, elevation, icing, no exercise, sprays, cleanses, follow-up appts. every single thing. Yet, I still can't breathe, my nose looks terrible, in my opinion...and I'm out thousands of dollars. I am bummed to leave a poor review because I truly did want it to be positive, but I can't at this time. The surgery failed, and now I'm left with very few options to actually get this fixed. Regretful is an understatement. Dr. Urbinelli is nice, did a good job with bedside manner, and the anesthesiologist did a great job. Unfortunately, the office staff and Dr. Urbinelli were unresponsive many times after surgery, but I gave them the benefit of the doubt. Overall, it is frustrating, mentally debilitating, and I'm at a loss for what to do next. If you're looking to get surgery done for breathing issues or reconstruction of the nasal area, I would suggest going to a nose specialist instead of a general plastic surgeon.
